C-R Staffing safety objective is to provide our employees with the safest working environment while on assignment with them, by training and educating them on safety awareness. Enclosed is a safety guide that you must read. Also, the contractor should provide the training and guidance necessary to eliminate unsafe practices and conditions at the job site. I understand and will comply with C-R Staffing and the contractors’ safety rules, regulations and hazardous communication.

Any employee who believes that a violation of a safety standard exists should bring the situation to the attention of their supervisor or the appropriate staffing coordinator. If corrective action has not been taken or a satisfactory explanation has not been given, you may request a safety inspection by sending a signed notice to the Department of Labor.

C-R Staffing utilizes a progressive discipline approach for violations of safety and health policies. This approach includes verbal and written warnings, suspensions and discharge. Discharge may be used as a first step, only for major violations.

Major violations, include, but are not limited to:

  • Offenses which risk others health and safety
  • Blatant disregard of safety rules
  • Possession, use or being under the influence of alcohol or illegal drugs

Contractor rules

I understand that some of C-R Staffing contractors may have additional policies, procedures, conditions, and security practices, and I agree to comply with those at will. If I do not adhere to these policies, I understand that this behavior may be cause for discharge.

Injury

If I sustain an injury on the job, I will inform my site supervisor and C-R Staffing immediately after the accident. C-R Staffing will deal promptly with legitimate claims and has workers compensation insurance that will pay medical expenses and wages, when applicable. C-R Staffing has extensive experience investigating claims and will fight fraudulent claims with all available resources.

An onsite representative will accompany me to the treatment facility provided to me by C-R Staffing, LLC. I will authorize and receive a post-accident drug screen. All bills, reports, and updates will be sent to C-R Staffing. I will keep C-R Staffing informed of my progress and I will inform C-R Staffing immediately of my release to work from the C-R Staffing appointed doctor. Failure to do so will indicate that I have voluntarily left the employment of C-R Staffing, which is an inactive status unavailable for further assignments. C-R Staffing will not be responsible for any injury in which I am performing tasks not authorized by C-R Staffing or the job site’s supervisory staff.

Medical information

I understand and authorize C-R Staffing to request and obtain all records regarding any industrial accident or occupational disease involving C-R Staffing and myself. This is to include doctor’s reports, follow up reports, nurse’s notes, medical bills, test results, etc.

Early return to work program

C-R Staffing recognizes the need to provide temporarily modified or light duty work to employees who are unable to perform regular duties due to work related illness or injury as soon as the treating physician deems it medically feasible. Due to the limited amount of modified work positions available, they will be assigned to by C-R on a “first come first serve” basis to those employees whose work related medical restrictions allow eligibility for the job positions available. 

Employees that have been subject to work related injury or illness would be returned to work as soon as the C-R Staffing attending physician approves their return in a capacity for which we have available work. We will make every attempt to secure useful and productive work for the employee within their specified work restriction. I understand that if I refuse an assignment that is within my work restriction, my workers compensation benefits will be terminated as of that day. Exceptions to this light duty policy include return to work under the Family and Medical Leave Act.

After 30 Days

This program will be for no longer than thirty (30) days per incident. Employees understand that if they refuse an assignment that is within their work restriction, their workers’ compensation benefits will be terminated as of that day. Exceptions to this light-duty policy include a return to work under the Family and Medical Leave Act.

Physician’s Approval

Employees that have been subject to work-related injury or illness would be returned to work as soon as the C-R Staffing’s approved physician approves their return in a capacity for which there is
available work. We will make every attempt to secure useful and productive work for the employees within their specified work restrictions.
